COVID Vaccines
FPC COVID-19 VACCINE UPDATE:
All Pennsylvania adults are now eligible to receive the COVID-19 vaccine. At Family Practice Center, PC, we currently provide the Moderna vaccine.
You may contact the Family Practice Center, PC location of your choice to learn when and where you can schedule your vaccine. Click here for contact information for all of our locations.
FPC Patients: If you receive a COVID-19 vaccine at a location outside of Family Practice Center, PC, please provide us with a copy of your vaccination card, so we can update your medical record!
My Medical Care
Please note: If you are having a medical emergency, call 911 immediately.
Contact your Family Practice Center, PC clinic directly by phone or via the patient portal to request a prescription renewal. We ask that you provide us a 24-48 hour notice. If you are overdue for a visit with your healthcare provider, you may be asked to schedule an appointment before we can renew your prescription.
Contact your Family Practice Center, PC clinic directly by phone or via the patient portal if you need to schedule, reschedule, or cancel an appointment.
If you are a new patient and wish to become established at Family Practice Center, PC, you may contact the location of your choice to make an appointment.
We accept new patients at most Family Practice Center, PC locations, and we participate with most major insurance plans.
If you have questions or concerns regarding your medical care, please call your Family Practice Center, PC clinic, or contact your providers using the secure patient portal.

We have healthcare providers on call at all times to assist our patients with acute medical needs when our offices are closed.

Adding donotreply@medentmobile.com as a contact in your address book will help prevent your portal email alerts from going to junk mail or being filtered out by your email settings.

We send medical records by first-class mail via the U.S. Postal Service. We cannot fax or email medical records. We cannot expedite medical records requests. They are completed in the order in which they are received.

You have the right to receive a “Good Faith Estimate” explaining how much your medical care will cost.
Under the law, healthcare providers need to offer patients who don’t have insurance or who are not using insurance an estimate of the bill for medical items and services.
You have the right to receive, upon request, a Good Faith Estimate for the total expected cost of any non-emergency items or services. This includes related costs like medical tests, prescription drugs, equipment and hospital fees.
To obtain one, simply call your local Family Practice Center office prior to receiving any services and let them know you need a Good Faith Estimate.
